COB

   - 6.15.17 COB Began Prepping  area for the stormwater project on my
   corner
   - 6.24.17 Began scraping Osborne rd asphalt and removed my tree and
   corner bush
   - 7.2.17 COB Broke ground
   - 8.17.17 Buried street asphalt in the yard and covered with hay
   - 8.22.18 email attached - Response to my complaints after 430 days of
   the initial concerns/emails etc
   - 10.3.18  Buried topsoil incorrectly

   - 2.9.22 Email attached COB told me it was my responsibility to fix
   their mishandlings on the corner


ATT

   - 8/8/18 surveyed/ dug up part of drain area and initial steps in
   removing the box
   - 7/17/19 fully removed ATT box
   - (trying to locate the date) They returned after the complaint to dig
   up the remaining underground wires ATT tried to abandon
   - 9/11/19 file date of Record Release
   - ATT Never fulfilled the stipulation section to repair the fence area
   on the original contract.
   - Yes, ATT created the drainage areas prior to removal and after removal
   - I have photos.

I have complete photo/video documentation of everything if needed.
